A blood panel for a synthetic patient: 14 results with units, reference ranges and flags, and nine patient identifiers marked in the truth file. Use it to test clinical extraction, and to check that a de-identification tool finds every identifier and leaves the results alone.

Two tests from one file: does your tool read the results, and does your de-identification remove the patient without touching the medicine?
No. The patient, laboratory and clinician are invented, and the phone number is in the 555-0100 to 555-0199 range set aside for fiction.
On purpose, yes. Low hemoglobin, MCV and ferritin point the same way, and MCV and MCH are calculated from the other values, so clinical NLP isn't tested on impossible combinations. Reference ranges vary by laboratory, and nothing here is medical advice.
Safe Harbor lists identifiers of the patient and of their relatives, employers and household members, not their clinicians. Many policies remove provider names anyway, so the truth file marks them separately and you can score either way.
Mask your real lab reports and notes, or generate synthetic ones with the true values. Prove it on 50 of your own documents in 48 hours.
Documents and text use the same policies, the same engine and the same deployment as these results.
